Your initial appointment usually will consist of a comprehensive medical history, and blood pressure screening, followed by an oral examination, head and neck cancer screening, screening for gum disease, necessary x-rays and review of your treatment options. Usually, treatment can be done or started the same day as the consultation. However, a more involved treatment plan will require a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time you schedule your first appointment:

• Whom we may thank for referring you to us.
• Contact information of previous dentist so we may obtain any current x-rays on your behalf.
• Inform us of as any allergies and/ or medical conditions.
• All insurance information, including: member id, group number, date of birth for subscriber and patient & 800 number for insurance company. Please keep in mind, we need at least 24 hours to adequately collect all your benefit information from your insurance company.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

Please bring a list of all medications you are presently taking to each appointment.

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.
